You must first conform to the Parents’ Hall of Fame Rules before your nominee is eligible for the Lifetime Achievement Awards. Please review the Parents’ Hall of Fame Rules in addition to these rules.
Due to the high volume of nominations and applications we will not be able to answer any questions about the receipt or status of any individual nominee (collectively, “Nominees”). All nominations and accompanying materials become the sole property of Family Awards and may be used in any manner in any media and will not be acknowledged or returned.
Note that “Parent” is used to denote the person who raised you, and who you are nominating by submitting this form. The nominee does not have to be a blood-relative.
Eligibility:
- In order to be eligible for the Lifetime Achievement Award, your parent must already be in the Parents’ Hall of Fame. Please read the Parents Hall of Fame rules in addition to these guidelines.
- You must be over the age of 18 to nominate your parent(s) for a Lifetime Achievement Award..
- You can only nominate your own Parent. Your nomination will be discarded if we find that you have nominated anyone other than your Parent (i.e., yourself, your sister, your best friend).
- Employees, officers, directors, advertisers, promotional and judging agencies and any other agents or affiliates of Family Awards as well as (a) the immediate family (spouse, ex-spouse, mother, father, sister, brother, daughter or son, regardless of where they live), and members of the same family (whether related or not) of such employees, officers, directors and agents; are not eligible to be in the Lifetime Achievement Award recipient or to participate in the application/nomination process. In addition, any applicant closely acquainted with any person connected with an employee of Family Awards is not eligible if, in our sole discretion, the person’s participation could create the appearance of impropriety.
- Nominee must not have ever had a restraining order entered against them. Nominee must not have ever been investigated for or charged with child abuse or child endangerment (whether convicted or not). Nominee must never have been convicted of a felony or misdemeanor other than a minor traffic violation.
- This award will not be granted posthumously so Nominee must be alive.
Process:
- All Nominations, once submitted, are the absolute property of Family Awards and may be used in any way that Family Awards decides in its sole discretion in perpetuity.
- All Nominations will be sorted by Male and Female nominees. A group of Semi-Finalists will be selected in a random drawing from each pool by a third party. The size of the group and the third party chosen to complete the drawing will be determined by Family Awards and is subject to change in Family Awards’ sole discretion.
- Each Semi-Finalist will receive a Semi-Finalist Package which will include the following: Medical History Form, Name and Likeness Agreement, Privacy Acknowledgement, Release Forms and Waivers for Semi-Finalist and their family members, Medical Release Form and Confidentiality Agreement. All Semi-Finalists must complete, sign and return the contents of the Package in order to be considered for selection to receive the Lifetime Achievement Award. Family Awards reserves the right to augment and/or change the contents of the Semi-Finalist Package.
- Family Awards will notify individuals by telephone if they are selected to be a Semi-Finalist. Family Awards or its agents will interview all Semi-Finalists in person and may conduct other interviews to gather additional information about the participant.
- The final choice for a Male Recipient and a Female Recipient will be in Family Awards’ sole and absolute discretion and may be made based upon any or all of the factors detailed in these guidelines. The decision may be subjective and Family Awards is not obligated to any nominee. Family Awards can remove, replace or reject any Nominee for any reason or for no reason.
- By submitting a nomination, each nominator agrees to release, discharge and hold harmless Family Awards from all claims and damages arising out of the nomination process.
- Prizes or rewards offered to nominees are subject to change without notice.
WHETHER AN APPLICANT IS CHOSEN AS A SEMI-FINALIST OR A RECIPIENT OF A LIFETIME ACHIEVEMENT AWARD IS AT THE SOLE DISCRETION OF FAMILY AWARDS.
RULES, ELIGIBILITY, DATES AND REWARDS ARE SUBJECT TO CHANGE WITHOUT NOTICE.
